MEDICAL RELEASE FORMS

Our current Medical Release Form is good through December 31, 2026.
If your information changes before then, please fill out a new form before your next trip. This form is required to go on any Capstone Church trip.

There are 2 versions of this form. One for Mission Trips and one for Non-Missions events and trips.

Use one of the links below to access the form. Once you have completed it and have had it notarized, please turn it in to your trip leader.
